COLLECTION AND USE OF USER INFORMATION
There are some areas on our Site in which we may request information from you. The personal information we request from you may include your:
Postal address (including billing and shipping addresses)
Date of Birth
(“User Information”). We ask for this information so that we can enhance your visit to our Site and provide you with information you expressly request. Your personal information is used only for the purposes for which you provide it. We take reasonable steps to ensure that information that we receive from you is reliable for its intended use, accurate, complete and current, including providing mechanisms by which you can amend or update your information.
The User Information you provide to us will be kept confidential and used only by us and any EA Partner to which you request we provide your information. We require that all EA Partners agree to use User Information based on our instructions and in compliance with all relevant laws regarding protection and use of your personal information and any other appropriate confidentiality and security measures. It will not be passed to any other party for marketing purposes. We may also share aggregated customer information such as statistics about our customers, sales, traffic patterns and related site information, with our business partners and other reputable vendors, but such aggregated information will not contain any personally identifying information of any users of the Site. We may disclose or access User Information when we believe in good faith that the law requires it.
Any information that Elite Aesthetics collects from you on the Site is information that you specifically and voluntarily provide. You may choose not to provide any or all of the information that is requested on the Site. In the event that you no longer want us to process or utilize your User Information, please contact us.

We may also collect other information. “Other Information” is any information that does not reveal your specific identity, such as:
Browser and device information
App usage data
Information collected through cookies, pixel tags, and other technologies
Demographic information and other information provided by you
We and our third party service providers may collect Other Information in a variety of ways, including:
Through your browser or device: Certain information is collected by most browsers or automatically through your device, such as your Media Access Control (MAC) address, computer type (Windows or Macintosh), screen resolution, operating system name and version, device manufacturer and model, language, Internet browser type and version and the name and version of the Services (such as the app) you are using. We use this information to ensure that the Services function properly.
Using pixel tags and other similar technologies: Pixel tags (also known as web beacons and clear GIFs) may be used in connection with the Services to, among other things, track the actions of users (including email recipients), measure the success of our marketing campaigns, and compile statistics about use and response rates.
IP Address: Your IP address is a number that is automatically assigned to the computer that you are using by your Internet service provider. An IP address may be identified and logged automatically in our server log files whenever a user accesses the Services, along with the time of the visit and the page(s) that were visited. Collecting IP addresses is standard practice and is done automatically by many websites, applications, and other services. We use IP addresses for purposes such as identifying your general location, calculating usage levels, helping diagnose server problems, and administering the Services.
Physical Location: We may collect the physical location of your device by, for example, using satellite, cell phone tower, Wi-Fi or Bluetooth signals, and other similar technologies. We may use your device’s physical location to provide you with personalized location-based services and content. We may also share your device’s physical location, combined with information about what advertisements you viewed and other information we collect, with our marketing partners to enable them to provide you with more personalized content and to study the effectiveness of advertising campaigns. In some instances, you may be permitted to allow or deny such uses and/or sharing of your device’s location, but if you do, we and/or our marketing partners may not be able to provide you with the applicable personalized services and content.
We may use and disclose Other Information for any purpose, except where we are required to do otherwise under applicable law. If we are required to treat Other Information as User Information under applicable law, then we may use it for all the purposes for which we use and disclose User Information.
INFORMATION COLLECTED BY THIRD PARTIES
If you request that one of the physicians in Elite Aesthetics’ network (“EA Partner”) contact you regarding certain Services contained on one of our Sites, EA Partners may collect additional Health Information from you. Any Health Information you share with the EA Partner is between you and the EA Partner. Elite Aesthetics does not have access to or store Health Information that you give to an EA Partner, except for such data that is disclosed by us to an EA Partner pursuant to a signed Patient Authorization Form. Elite Aesthetics is not responsible for the security, integrity or maintenance of such Health Information disclosed to an EA Partner.
We take appropriate measures to keep the personal information you disclose to us secure and to only release this information to third parties that share our commitment to privacy. However, we cannot guarantee there will not be a breach and we are not responsible for any breach of security or for the actions of any third parties.
USE OF THE SERVICES BY MINORS
The Services are not directed to individuals under the age of eighteen (18). These individuals may not provide User Information or Health Information through the Services.
